I hereby consent and authorize an agent of the Seventh Day Adventist Reform Movement (SDARM) to take photographs and/or motion pictures of me; or to produce video, web casts, or other types of media productions that capture my name, voice, and/or image (any of the foregoing types of media are called the “Materials” in this Consent and Release form).

I authorize SDARM to copyright the Materials, and I authorize SDARM to use, reuse, copy, publish, display, exhibit, reproduce, and distribute the Materials for any promotional or marketing purpose, as well as other publications or other forms of media, which may include, but are not limited to SDARM publications, catalogs, articles, magazines, brochures, websites, social media, including Facebook, Instagram, Twitter, etc., electronic or otherwise, without notifying me.

I further acknowledge that my agreement and participation is voluntary, and I understand that I will not receive financial compensation of any type associated with the taking, use or publication of the Materials of any kind. I acknowledge and agree that publication of Materials, in any manner or medium, confers no rights of ownership or royalties whatsoever to me. I hereby release and hold harmless SDARM, its Department Members, its contractors, and any third parties involved in the creation or publication of marketing materials, from any reasonable expectation of privacy or confidentiality associated herewith as well as liability for any claims by or through me in connection with my participation.
